Population Health Promotion: An Integrated Model of


Population health is an approach to health that aims to improve the health of the entire population and to reduce health inequities among population groups. In order to reach these objectives, it looks at and acts upon the broad range of factors and conditions that have a strong influence on our health.

Healthy communities & population health


The Population Health Team in the Office of the Chief Medical Health Officer provides leadership and expertise on population health initiatives. The team is headed by a regional director and includes policy consultants, knowledge translation and health promotion specialists. Many VCH staff use a population health approach, including the Medical Health Officers and community health staff.
